It is used to extend the rubber isolators that connect to the tailgate and are designed to hold tight against the spare. You can try the daystar ones as well (I have them) but they kind of suck.

I agree on the Daystars sucking.
You'll want radiator hose that has the same inside diameter as the outside diameter of the factory rubber isolators. you simply cut the hose what ever length you need to go from the tailgate to the side wall of your spare, and then slip it over the factory isolator.

My 315/70 17 duratrac did not even come close to fitting on my stock carrier with the stock bumper. I have an aev pintler wheel 5.2 backspace. I had a generic bracket that moved the wheel up maybe 2" and out 2" but then it stuck out too far for my taste even with extended bumpstops. I ended up buying just the teraflex tire mount, it allowed me to adjust the tire right against the factory bumpstops.
Depending on the backspace of your wheel you may be able to get away with a standard bracket (like I had originally) like many manufacturers offer for around $40. Smitty, teraflex and others offer them, the expedition one bracket looks nice but is expensive for a spacer. And if any standard spacer doesn't give the fit you want it is nonadjustable.

I have the exact setup and the wheel/tire combo would not fit on my stock tire carrier... Bad pics but it will illustrate that the smittybilt oversize carrier works great for the price and is a solution
