I know it isn't exactly a bumper/tire carrier combination, but have you looked into the OR-Fab tire carrier?

I agree with B1000, OrFab tire carrier with LoD rear bumper. Especially since your not going bigger than a 34. That's what I'm running.

For me, and that's what I got since I was in the same situation a few months back, the best is the shorty LOD gen 3 with carrier independent of the tailgate.
Not being linked allows you to carry more stuff (like the trail rack, which is very useful specially for 2 doors), and the fact that if you happen to back into something like a tree or boulder, you will not damage the tailgate, since all of the impact will be taken by the bumper.

Voted other - Ace Rear Bumper and Tire Carrier. Ace gear is solid and well priced and you can usually find 10 - 20% off sales. The attached pic is the 2nd gen but they now have the 3rd gen out which is even better.

Or-Fab Rotopax Carrier with Smitty XRC Bumper. Holds the 37 with no problem.
