get a 6LN (6 inch long nose) set of vise-grip pliers. A NEW SET, around 10 bucks. Hold the nutsert around the head , get an impact wrench and zip the bolt out. An impact will ''break'' the bolts' hold on the nutsert...A new set has sharp teeth that will grip without crushing the nutsert head....
