Default Control Arm Issue

I had a 3" teraflex lift installed on my jeep a little over a week ago. After walking outside to day i noticed the control arm on the driver side was turned to the side a bit. After investigation i found that a screw on the top fell off after only touching it, also it appears there is washers that are missing compared to the other side. I am posting some pictures for anyone that is willing to help. My question is was is the issue here and what needs to be done to fix it.

Take it back to the shop who installed it. The screw is a grease fitting not a huge deal that it fell off. The joint looks like it wasn't properly tightened to the actual arm or the frame itself

The grease fitting, just take a wrench and throw it back on there. As far as the joint, the only thing that will be done is loosening the jam nut, aligning the joint, then tightening down again.

I prefer to have the zerk facing up. When it hangs down you are bound to break it off on something.

And that looks like a 4 wheel parts quality install.
