Why use an impact gun anyway this isn't a dump truck. Glad everyone is ok though.

that could happen to anyone, spacers or not, if you overtorque the lugnuts.

I think it's probably even better to say that the mechanic failed. The spacers didn't cause this and I'm willing to bet the studs didn't either. The assclown doing the work caused it, that's most often the case.

Last time my Jeep was at the dealer the mechanic came out and told me "Make sure you check your lug nuts when you get home. I had a lady come back and bitch because she lost two lug nuts on the way home. My impact gun isn't working properly."

USE A TORQUE WRENCH.

Good point. but i have had bad studs in the past.

This is correct. The spacer did not fail. The lugs were WAAAAAAAAAAAAAAAAAAY OVER TIGHTENED!!!! I took off all of the wheels today and most of the lugs are stretched. I am going to buy 4 more spacers and start over. If I didn't have spacers the same thing would have happened to the factory lugs.

i dunno if you have a press, but you could just buy 20 new studs and replace all the studs?
